If requested Consolidated Social Work Services will only accept copies, that are made a part of office records on your behalf, and only after you have signed a request for assistance form, and a release of information form. These copies will not be returned to you. This statement is contained within the release of information form you are asked to sign. The signing of this document does not mean we have accepted your case, only that as resources become available this orgganization will review your case to see if it fits within the discription of what this organization does.
